Sometimes people who try too hard to unit test are just as bad as people who don't unit test at all.

I don't see a clear WTF in this other than the company not having a test environment for Jack's "Proof-of-Concept" / learning demo. And the fact that no one took the time to figure out what was wrong with the uploads.

Everything about Jack was a WTF from end to end.
An all too common sort of WTF, but as we've seen over the years here, every episode is just a variation on the theme of rampant incompetence and under-resourcing from the lowliest QA worker or install-monkey to the most exalted CIO.
