    Doesn't your list change depending on where you are? From what I've read about the nanp some areas (overlays) must use 10 digit dialling, or do they still need the 1 so [2-9] would be an error?

    But yes an international number can be anywhere between 7 and 15 digits, including country code, excluding access code.

    Typically, yes. "You must first dial a 1, then the area code".

    That would be forwards compatibility to mobile cellular phones, since the POTS accept DTMF tones only. It's a UI thing since cellular digital networks use out of band signaling so DTMF is not used for connection (maybe during a call to an IVR). But if it's part of DECT maybe it's required for ISDN? I don't have experience in that area.

    then it is in band but not really used for connecting, even when the IVR transfers. The IVR will just collect the DTMFs and transform that info in a number to dial on that line or a secondary line.

    Almost always in-band. ISDN wasn't designed for communicating much meta data about the calls it carries.

    Not for cellphones; maybe some, when they were still operating on analog channels, but they're not even using DTMF for direct dialing anymore. It's all digital.
    So the real problem is that they have to know when to switch to DTMF so that you can navigate the automated system that you've just called.
